Fees and Payment
|
Bartky HealthCare Center LLC accepts private pay only. We are happy to provide you with an itemized bill
suitable for presentation to your insurance carrier but are not in any way party to any arrangement you
may have with your insurance carrier.
You are responsible for payment at the time of each visit. Visits may be paid for by cheque, credit card
or cash. Cheques should be made payable to "Dr. Eric Bartky".
We require 24-hour notice of cancellation for all appointments, including initial evaluations.
An advanced payment is required to secure all first visits.
Please review your personal schedule carefully in order to be able to make a commitment to your office
appointment times. As much as we would like to be able to offer you a timely appointment after you have
cancelled, we often cannot. We have waiting lists that must be honored. When you do not keep an
appointment, and do not call us with 24-hour advance notice, it is not only a loss of income to the
practice, but also a loss of valuable treatment time that could have been offered to someone else.
Under certain circumstances, additional non-emergency clinical or case management telephone contact
time may be charged.
